First Visit

Your "New Patient Experience" will always begin with a thorough examination and diagnosis. After completing a thorough exam with Dr. Barwick, we will work together to develop a plan that is essential to your dental health.

Please assist us by providing the following information at the time of your initial visit:

R Any x-rays from your previous dentist. We'll be happy to request these for you.

R Your completed health history, and a list of medications you are presently taking. Please alert our office if you have a medical condition that may be of concern in your treatment. Also let us know if you are taking any special medications, or if you require medication prior to your dental cleanings (e.g. antibiotic pre-medication).

R Your dental insurance plan details. If you have a dental insurance plan, please bring the nec-essary information.

IMPORTANT: A parent or guardian must accompany all patients under 18 at their first visit.
